• Interactions
  • Lactoferrin binds iron strongly (Fe³⁺) and can therefore reduce the availability of iron in the lumen of the intestine. This can decrease the absorption of oral iron supplements if taken at the same time; Therefore, it is advised to keep at least 2 hours between lactoferrin and iron preparations. At the same time, lactoferrin can modulate inflammation and infection-related calibrator distribution, which is desirable in some clinical contexts.
  • Due to its immune-modulating effect (e.g. influencing TLR signals, cytokine profile and neutrophil activity), lactoferrin may slightly alter the response to infections and possibly to immunomodulatory medications. There is no hard data that lactoferrin inhibits or induces specific CYP450 enzymes, but it does alter the mucosal microenvironment, which may affect the local availability and sometimes tolerance of oral medications.
  • Lactoferrin may support the effectiveness of some antimicrobials by disrupting biofilms and limiting pathogen growth via iron extraction. This synergy has been described in certain antibiotics and antifungal drugs, but never replaces regular therapy. In patients with complex hematological disorders or iron overload (hemochromatosis, transfusion overload), combination use with other iron-modulating agents deserves additional monitoring.
